Message from University Health Service
To: Students, staff and retirees who are eligible for primary care at
the UHS
Self-pay Dental Scheme
The University has appointed three outside dental groups to offer self-
pay dental plans on preferential rates to students, staff and retirees,
to supplement the existing dental services provided by the Dental Unit
of the University Health Service. This self-pay dental scheme serves
to provide an additional option for members in receiving dental
treatment outside the campus, thereby giving more flexibility and
quicker accessibility.
Eligibility
2. This self-pay dental scheme is open to:
(a) Students eligible for primary care at the UHS;
(b) Staff eligible for primary care at the UHS and their immediate
family*;
(c) Retirees eligible for primary care at the UHS and their
immediate family*; and
(d) Staff on fixed term contract who have reached the
retirement age and have served the University for more than 10 years,
and their immediate family*.
* Immediate family refers to spouse and children. The dental groups
agree to cover all children irrespective of whether they are defined as
dependent children by the University.
Coverage
3. The three outside dental groups, listed below, offers an
annual dental plan which covers oral examination, scaling and
polishing, filling, extraction, at a prepaid fee:
(a) Bayley and Jackson Dental Surgeons Ltd.
(b) Quality HealthCare Dental Services Ltd.
(c) UMP Professional Management Ltd.

致：合資格使用大學醫療保健處服務之學生，僱員及已退休僱員
自付牙科保健計劃
大學已委任三間校外牙科服務機構以優惠費用為大學的學生，僱員及已退休僱員提供自付牙科保健計劃，以支援大學保健處牙科診所現有服務。本計劃的目的是為提供於校園以外另一項更俱彈性、更快捷的牙科服務選擇。
資格
二、下列人士皆可享受本自付牙科保健計劃：
（一）合資格使用大學醫療保健處服務之學生;
（二）合資格使用大學醫療保健處服務之僱員及其直系親屬*;
（三）合資格使用大學醫療保健處服務之已退休僱員及其直系親屬*; 及
（四）已達到退休年齡及在大學任職超過十年的指定限期合約的職員，及其直系親屬*。
*直系親屬是指配偶及子女。牙科服務機構同意所有子女均可參加本計劃服務不須符合大學定義的須供養的子女。
涵蓋範圍
三、 下列的三間校外牙科服務機構，將會提供以預付形式的年度牙科保健計劃，服務包括口腔檢查，洗牙和淸除牙石，補牙及脫牙等：
（一） 庇利積臣牙科醫務所
（二） 卓健牙科服務有限公司
（三） 聯合醫務專業服務管理有限公司。
